The Trendelenburg gait is typically caused by weakness in the hip abductor muscles, making it difficult for the hip to support the body’s weight on the affected side. Normally, during walking, the pelvis tilts upward on the side that isn’t bearing weight, but in Trendelenburg gait, the pelvis drops on the unsupported side. This can lead to leaning over the affected hip to compensate.
Hip replacement surgery is a common cause due to unintentional leg length changes or weakness in the buttock muscles. Other causes include congenital hip dysplasia, arthritis, muscle tears, tendinopathy, or nerve damage.
Treatment involves strengthening the hip abductors through physical therapy, including exercises with weights or resistance bands. Lateral step-ups, closed-chain exercises, and balance exercises are recommended. If leg length discrepancy is the cause, orthotic intervention may be necessary.
Pelvic support, physical therapy, and avoiding prolonged sitting are recommended for managing the condition. A waddling motion may indicate Trendelenburg gait, and proper treatment is essential for correction.
